Clinical History:

36-year-old male with right abdominal pain and hematuria. 


Findings:

( 1 and 2 ) Gray scale and color flow imaging of the right kidney shows an anechoic area in the renal sinus, which fills with color.

(3) Corresponding contrast enhnaced CT shows intense enhancement of this renal sinus lesion ( equal enhancement to aorta).

(4) Renal angiogram confirms presence of a large arterio-venous malformation.

Discussion:

Arteriovenous malformation (AVM) is one of the commonest reno-vascular lesions which may mimic malignancy. It may be congenital or acquired, common causes being trauma (most common), surgery, tumors, or idiopathic.
Arterio-venous fistulas comprise 70-80% of AVMs in the kidney. Renal AVMs may be located within the renal parenchyma or in the renal sinus, and therefore may be difficult to be distinguished from a renal malignancy such as a transitional carcinoma.

Color flow Doppler may prove useful in making the diagnosis by depicting a typical arterio-venous flow pattern in the lesion
