Clinical History:

 Figure 1: Axial noncontrast CT image shows herniated right pelvic mass through the posterior and medial thigh region via obturator foramen. Mass is composed mostly of fatty tissue (-150 HU) with internal streaks.

Figure 2: Distally, the mass contains nodular lesion that isodense with muscle tissue.


Findings:

A 4-year-old girl was admitted to paediatric surgery clinic for longstanding, palpable mass at the right thigh.


Diagnosis:

Discussion:

Teratomas, by definition, are tumors that are composed of multiple tissues containing at least two germ-layer derivatives foreign to the part of the body in which they arise. Ovarian teratomas comprise about 37% of all teratomas in infants and children, and are second only to sacrococcygeal as the most common site for teratoma (1).

This is a fabulous case. I have seen many obturator hernias but not a case with mature cystic teratoma herniating through the obturator foramen.

Congratulations on submitting a very rare case of herniation of ovarian teratoma.

Such kind of presentation of ovarian teratoma has never been reported so far in any of the pubmed indexed journals.

very rare case. So little girl with such a big teratoma. I believe that nothing is impossible. I saw teratoma but never saw teratoma with herniation. Thank you.
